Build an intake form
An intake form collects what you would otherwise spend the first ten minutes of a session asking, and puts the answers on the client record instead of in an email you have to transcribe. Start from a template rather than a blank builder; the templates exist because the same questions come up in every practice.
How to do it

Open Forms, then browse the templates
Pick the closest one and adjust it. Building from scratch is available but rarely the fast route.

Edit the questions
Add what your discipline needs, remove what it does not. Anything you would not read before a session is a question worth cutting.

Share the form
Forms have a public link a client can complete without a login, and they can be embedded in your own website.

Read the responses
Submissions appear under Forms, and against the client they came from.
Sending it automatically
Once the form exists, an automation can send it the moment a booking is made, so you are not remembering to email it to every new client. That is usually the point at which intake forms stop being a good intention.
