Soap Notes + Fathom

Turn a recorded session into a written note you still author.

Fathom records and transcribes your calls. Connect it to Soap Notes and those recordings line up against the matching booking, so the session note and the recording of that session sit in the same place. From the note you can drop in Fathom's summary, its action items, or a chosen stretch of transcript. Everything lands at your cursor and adds to what is already there, so the note stays a document you wrote rather than one that filled itself in. That distinction matters when the note is a clinical record.

How to connect Fathom

    Connect Fathom

    Settings → Integrations → Fathom → Connect. You are sent to Fathom to sign in and approve access, then returned to Soap Notes. Fathom exposes a single scope, public_api, so that is the whole of what you approve.

    Record a session as you normally would

    Nothing changes in how you use Fathom. Once a recording finishes processing, it appears in Soap Notes against the booking whose calendar invitee matches the meeting.

    Open the session note and insert what you want

    The recording appears above the note editor and in the Recording panel. Choose the summary, the action items, or highlight part of the transcript, then insert. The text lands where your cursor is.

Frequently asked questions

No. Every insertion happens on an explicit click, and it adds to the note rather than replacing what is there. The note stays yours, which is the behaviour you want when it forms part of a clinical record.

Link it to the right booking by hand, or unlink it. A recording that has not matched anything yet sits unmatched until you place it, so nothing is guessed into the wrong client's file.

Per recording: the title, Fathom's summary, the transcript as speaker-attributed lines with timestamps, action items with their assignee and playback link, the share link, the host's email, and the start and end times.

New recordings stop arriving. Anything already inserted into a session note stays, because at that point it is part of the note rather than a link to Fathom. Reconnect whenever you want to resume.

The Fathom client ID has not been set on your deployment. It is a configuration step on our side rather than anything wrong with your Fathom account. Contact support and we will sort it.
